Very well done mate. As a suggestion, it might be a good idea to apply some back emf protection to your op amp output. When you are using inductive loads like a motor, if you get a worn brush on the motor or the motor gets disconnected for some reason, you will get a massive voltage spike which will kill your device. Normally you would use a diode wired in reverse across the motor windings.
