Apparently the reason the final level in Postal was a school was because the game devs believed it was an extreme length no gunman would ever think about going to. In the level, you don’t even kill anyone and just have a mental breakdown. Two years later Columbine happened and prompted the devs to change the final level to a park instead.
I like how the severity of these games is essentially 1. Regular game that's just a bit too graphic 2. Sexual violence 3. Made by a terrorist organization

Here's an honorable mention that should be included in a potential part 2. The Punisher. This was a 3rd person shooter that was released in 2005 and developed by Volition. Players take control of the titular character as he wages his one man war on crime. The game is notable for being extremely violent, with some scenes depicting the Punisher interrogating and torturing criminals in brutal ways using the environment. The game also has some brutal executions as well. Due to these, the game had to be censored to avoid getting an AO rating in the US. However, these censorships did not prevent the game from being banned in certain countries like Australia and Germany.
