I was taught to never say someone was intoxicated. To instead refer to them as impaired. Simply because Diabetic Keto Acidosis can easily mimic and present as someone that seems to be intoxicated, confused, difficulty paying attention, flushed skin, nausea, vomiting, and a sweet alcohol or acetone smell to the breath. Also brain injuries such as an active TIA, CVA, a head injury obtained in a MVC, or a previous head injury, dementia, seizure disorder, ataxia, allergies, fatigue, and even a rare metabolic disorder called Auto Brewery Syndrome...all can present as if the patient is intoxicated and cause noticeable impaired coordination, slurred speech, delayed responses, difficulty with concentration, and confusion. Even breathalyzers can produce false readings with a patient that has not consumed any alcohol with DKA and the Auto Brewery Syndrome. So unless someone on scene can definitely diagnose the cause of the patient appearing intoxicated as that being the truth, the legal way to refer or document the patient is as impaired. That way no legal repercussions of any negative manner can be assigned to you as a responder.
