If you had a moderate bad year (a 3.0) and three other years holding a 4.0 (which is extremely difficult), your average still only comes out to 3.75. A 0.25 decrease means a lot for any prestigious medical school
Sounds like someone had "one bad year" in math. Outliers have a huge impact on the mean, especially with only 4 data points
