Agile has been popular among tech companies for many years, but starts to be common knowledge among most of the companies, with many articles describing how these big tech companies are succeeding partly because of Agile. So, why Agile fails, why is it that many companies, even Tech ones, are failing to adopt Agile?

Culture is going to play a big part, and Agile is fundamentally a big culture change, which will make it difficult to adopt, since, most likely it will be a quite different culture from the one existing previously in the company.

Technical teams have been more comfortable to adopt Agile, but taking it into other teams in the company remains a challenge.
