The Care Manager homepage consists of a streamlined set of tasks including patients by program, as well as anything on a care manager’s calendar. When the care manager opens any task or assessment, it will automatically display on the right and the full longitudinal patient record will display on the left. This means your staff doesn't have to hunt for information — it's right there: where and when they need it.
Based on the outcomes and assessment, additional task care plan activity and future assessments can be automatically scheduled, as well care plans shared with patients as appropriate. And your entire care team can see the same patient and all the details around their programs in real time. This information can also be displayed at the point of care through Arcadia’s desktop application.
