#27 Did you Know? Get what you need before signing those discharge papers

The hardest thing to do is try to coordinate care after your hospital, ED, or same day surgery discharge. Ask for copies or orders placed, a direct contact number and name for any questions, and documentation of "when" the need will be fulfilled. The same goes for any prescriptions for any need (e.g. medication, equipment, food, etc.) ensure it has been sent to the receiving entity, and again ask for a direct name and their contact number should you need to do follow-up.
